The USRFE trace record represents abnormal termination of an access
method routine for basic sequential access method (BSAM), queued sequential
access method (QSAM), basic partitioned access method (BPAM), or basic
direct access method (BDAM).

- Return code from the module that detected the error condition.

- Time (hour.minute.second) when the GTRACE macro was processed
or blank, if the time is not available.
