A disk I/O may fail if a node is removed from an SVC cluster while the AIX image is sending I/O to the SVC node. This error will only occur with the AIX default PCM and SVC devices.

Problem conclusion
Modified AIX PCM to better handle the SCSI Long Busy status that is returned when a node is removed from the cluster.
