Notes or Domino crashes with error "0x115 - RTVSCAN is operational and may be holding our memory, exiting..."

Technote (troubleshooting)

Your Notes client or Domino server crashes with the following error: "0x115 - RTVSCAN is operational and may be holding our memory, exiting. Please restart RTVSCAN or reboot the system".

When Domino, Notes, or any Notes API exits, the program tells the operating system (OS) to destroy the shared memory. The problem arises when the OS does not destroy it as long as a single process is attached to Notes/Domino shared memory.
Many times, we detect that RTVSCAN is running and the Notes/Domino memory is still around because RTVSCAN is holding it. So, we print out this "specific" message on the server or client.

If Notes/Domino is already running and RTVSCAN attaches to it, then there is no issue. Tthe problem arises if RTVSCAN is holding Notes memory at the time of starting or stopping Notes/Domino.
