• Steps to replicate:
  
  1) Configure map manager record to use this web map

  2) Clear browser cache
  3) Go to any spatial-enabled application
  4) Open Spatial Layers Tool from Open Map DIalog box.
  
  Actual Results: map toolbar is displayed duplicated
  
  Expected: map toolbar displays expected maptools correctly

Local fix

• Close Open Map Dialog Box and open again. After first access
  map toolbar is loaded correctly.
