IJ37350: DEADLOCK DURING THE AFM PREFETCH

Error description

• AFM prefetch might get stuck during the queuing
  phase if the list file has duplicate entries. This happens
  because a waiting thread is not notified after the read
  completion.

• Remove duplicate entries from the list file
