IBM Explorys Platform with Enterprise Performance Management (EPM) enables healthcare systems to collect, link, and combine data from hundreds of disparate sources across the enterprise and clinically integrated networks. These data sources span major types, including clinical electronic medical records, practice management system, adjudicated payer claims, billing, accounting, device, community, and patient reported outcomes. This collected, linked, and combined data is referred to as the healthcare interprise, since it represents the convergence and standardization of big data that resides inside and outside of an organization. IBM Explorys Platform with Enterprise Performance Management (EPM)

IBM Explorys Platform with EPM is a set of software as a service (SaaS) based applications designed to enable rapid population management, performance measurement, and effective engagement and outreach to patients and providers.

IBM Explorys Platform with EPM maintains a connection to a multitude of clinical, operational, and financial data sources, including clinical electronic medical record (EMR) systems, primary care office practice management systems (PMS), healthcare billing systems, and private and public insurance plan and payer claims feeds. Data is standardized, normalized, and harmonized to create a unified longitudinal patient record connecting all data sources and venues of care.

IBM Explorys Platform with EPM harnesses the power of big data. IBM Explorys Platform with EPM provides reporting, business intelligence, data analytics, and population performance management across the continuum of care. IBM Explorys Platform with EPM applies the power of massively parallel data processing to help improve patient treatment and make healthcare more affordable. IBM Explorys Platform with EPM focuses on the transformation from fee-for-service to the new value-based care payment models. IBM Explorys Platform with EPM has capabilities in place designed to safeguard protected health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH).

IBM Explorys Platform with EPM is a combination of technology and prescriptive programs for healthcare transformation. This combination helps healthcare organizations to bridge the data gap across each venue of care and is an increasingly diverse delivery model. IBM Explorys Platform with EPM includes enterprise security standards, robust data collection, encryption, curation, and governance necessary for operating in complex collaborations as well as a state-of-the-art attribution engine for aligning healthcare organizations with patient panels, registries, and performance metrics. IBM Explorys Platform with EPM also unifies and enables applications, both from IBM Explorys Platform with EPM Application Suite and third parties, to deliver powerful insight into at-risk populations, performance, and addressable risk for managing even better outcomes, and helps identify potential cost reduction strategies.

IBM Explorys Health Data Gateway appliances

IBM Explorys Health Data Gateway appliances are delivered by IBM in pairs to provide for uninterrupted transmission, data acquisition, and to monitor and transmit data. These appliances are owned by the IBM Corporation. They transmit data from the point of care to IBM Explorys Platform with EPM.

IBM Explorys EPM Application Suite

IBM Explorys EPM Suite empowers healthcare stakeholders to measure performance of their accountable providers, and the patients and members they manage. The suite facilitates identification of suboptimal performance and care gaps across hundreds of standardized quality metrics, including those needed for national programs from the Centers for Medicare and Medicaid Services (CMS), National Committee for Quality Assurance (NCQA), among others, and managed care coordination programs in order to predict outcomes around cost and utilization, and allows users to identify potentially avoidable events.

IBM Explorys EPM Suite is available as an annual subscription and has six core applications included in the base product.

  • IBM Explorys EPM: Inform contains score cards that have configurable data visualizations so you can share performance metrics. The scorecards empower you to more effectively drive successful population health through intuitive visualizations powered by cognitive learning to align you as the right user with the right insight at the right time.
  • IBM Explorys EPM: Measure is an integrated framework for correlating billions of clinical, operational, and financial events into action-driven benchmarks and scorecards for comparison at the enterprise, department, and patient levels. It is designed to serve multiple roles and audiences within a healthcare organization. IBM Explorys EPM: Measure enables senior leadership, quality officers, care coordinators, and individual providers to establish benchmark targets, view historical performance against those targets, and take action based upon specific information.
    • Starter Pack Libraries for IBM Explorys EPM: Measure includes data visualization features such as historical trend lines, comparative bar graphs, and motion charts that provide a multidimensional representation of data to reveal trends and correlations in performance. Starter Pack Libraries categories include health and wellness, operational metrics, and disease management for diabetes, heart failure, hypertension, and coronary artery disease.
    • Measures Highlighting and Evidence Highlighting provide an easy-to-read intuitive interface to comprehend complex information related to performance management. Measure Highlighting provides critical measure information in an easy-to-read format that focuses on key aspects of the measure, such as denominator, numerator, exclusions, exceptions, and reference period. Evidence Highlighting goes beyond the measure itself to understand the results of the measure in a concise and understandable format. Seeing the patients and correlating system identifiers in each aspect of the measure provides key insights on the results of the measure. This saves on analysis time and provides a clear evidence profile for the measure.
  • IBM Explorys EPM: Registry is integrated framework for producing criteria-defined patient lists accompanied by extensive clinical, financial, and operational data. Explorys EPM: Registry allows you to quickly identify your target population and view data that empowers decision making and risk stratified care management. The matrix of rows (patients) and columns (data elements) can be added, removed, sorted, or filtered as needed.
    • Starter Pack Templates for EPM: Registry includes three registries: diabetes, heart failure, and hypertension.
  • IBM Explorys EPM: Engage is an integrated solution that plugs into other applications within Platform with EPM Application Suite for managing patient work lists and facilitating workflow management.
Prebuilt analytics
Registries: Fifteen core registries are available within IBM Explorys Platform with EPM, with the capability to create more. The patient lists are defined by specific criteria that are configurable based on initiatives and specific customer needs.
Risk models: Thirty core risk models are available within IBM Explorys Platform with EPM with the capability to create more. A risk model is a predictive analytic model that quantifies the risk associated with event, outcomes, or cost. IBM Explorys Platform with EPM has the ability to support commercial and publicly available risk models as well as the IBM Explorys Platform with EPM proprietary risk models provided.
Measure sets: Seventy-five measure sets are available within IBM Explorys Platform with EPM. The measure sets are tools that help track the quality and cost of health care services provided. The measure sets can be defined by public National Committee for Quality Assurance (NCQA) or configurable based on the client requirements defined by the National Quality Forum (NQF) or Centers for Medicare and Medicaid Services (CMS).
  • SuperMart includes a set of prebuilt tables and schemas designed to meet the demands of value-based care, analytics, and reporting. SuperMart is used to support existing business intelligence tools and frameworks, as well as applications that can benefit from curated data contained within it. This is a production system with access directly to the SQL database. In addition, a SuperMart Extension is available as an add-on application that provides an analytic sandbox for users to extract, transform and load (ETL) their datasets to SuperMart regardless of source.

IBM Explorys Population Assessment

IBM Explorys Population Assessment, an analytic offering, allows a healthcare system to effectively manage utilization and health of a value-based care population of interest. It is delivered as a PDF report along with an analytic dataset that comprises the underlying data behind the report. Using this solution, providers can better understand the pertinent characteristics of their target populations and identify the right initiatives to better manage the health of their patients. IBM Explorys Population Assessment is a cost-effective solution that lets providers more efficiently manage any population in accordance with the Institute of Healthcare Improvement's Triple Aim Initiative by improving the patient experience, improving the health of populations, and reducing the per capita cost of healthcare while significantly shortening time to value. There are three offerings available for IBM Explorys Population Assessment:

  • Medicare Advantage
  • Medicare Shared Savings Program
  • Commercial

IBM Explorys Population Assessment Investigator Watson Edition

IBM Explorys Population Assessment Investigator Watson Edition provides individuals and teams the ability to perform analysis into factors driving high risk or utilization without the need to understand the programming schema or SQL language. The Investigator is an integration of IBM Watson Analytics combined with the population assessment analytic dataset. The Investigator provides cognitive insights accelerating your ability to manage at risk populations more effectively.

Data regulations and governance

With the governance engine, you can get rules-based access to data based upon the client's data governance policies and conditions, such as organizational role, patient attribution, and contract rights between entities. This enables patient data to be made available to various personnel and functions in appropriate ways consistent with a client's business rules and identified regulatory policies. It also enables the rapid formation of relationships between payers, providers, and plans as models grow and evolve.

All data that enters any aspect of the IBM Explorys offering does so through encrypted data channels using a virtual private network (VPN) that has been established between the Explorys data center that is called the Patient Data Vault (PDV). The PDV is a specially encrypted database (MySQL) that utilizes Advanced Encryption Standard (AES-256) disk encryption. During runtime, IBM Explorys EPM: Measure accesses the PDV using client certificate web services. Access to the PDV and its web services are restricted to registered applications, such as IBM Explorys EPM: Measure and are not available to users directly.

The data includes diagnoses, procedures, medications, laboratory results, implantable devices, patient reported outcomes, vital signs, encounter-level data, providers, and other clinical, financial, and operational data. Treatment information is available at the pharmacology class, ingredient, brand, and generic level. The dataset also contains information on drug strength, dose, and delivery form.

Instance: Instance is a unit of measure by which the Cloud Service can be obtained. An Instance is access to a specific configuration of the Cloud Service. Sufficient entitlements must be obtained for each Instance of the Cloud Service made available to access and use during the measurement period specified in Client's Proof of Entitlement (PoE) or Transaction Document.

Item: Item is a unit of measure by which the Cloud Services can be obtained. An Item is an occurrence of a specific item that is processed by, managed by, or related to the use of the Cloud Service. Sufficient entitlements must be obtained to cover every Item processed by, managed by, or related to the use of the Cloud Service during the measurement period specified in a PoE or Transaction Document.

Health Care Provider: Health Care provider is a unit of measure by which the IBM SaaS can be obtained. Health Care is the maintenance or improvement of a person's physical or mental health. A Provider is a Health Care institution or person of a Health Care team rendering Health Care services. Sufficient entitlements must be obtained to cover the total number of Health Care Providers managed, processed, or tracked by the IBM SaaS during the measurement period specified in Client's PoE or Transaction Document.

Terabyte: Terabyte is a unit of measure by which the Cloud Service can be obtained. A Terabyte is 2 to the 40th power bytes. Sufficient entitlements must be obtained to cover the total number of Terabytes processed by the Cloud Service during the measurement period specified in Client's PoE or Transaction Document.

Thousand Individuals: Thousand Individuals is a unit of measure by which the IBM SaaS can be obtained. An Individual is a single unique person. Sufficient entitlements must be obtained to cover the total number of Individuals, rounded up to the nearest one thousand, managed, processed, or tracked by the IBM SaaS during the measurement period specified in Client's PoE or Transaction Document.
