Attempts to navigate to the registered IBM Rational Change and Configuration Management application from the IBM Rational Jazz Team Server results in the error "The application could not be reached".
After successfully registering the Change and Configuration Management application to the Jazz Team Server, the link to the Change and Configuration Management application does not show up in the Jazz Team Server home page.
The User Interface shows an error of:
Change and Configuration Management (CCM) (0)
The application could not be reached.
However, when viewing the 'Registered Applications page', the Change and Configuration Management application is listed as a registered application of the Jazz Team Server.
The following error is found in the server log:

The Change and Configuration Management application hostname or port number are not defined as a virtual host.
Resolving the problem
In Rational Team Concert 3.0.1 and prior, the hostname and port number used by the Change and Configuration Application must be configured as a virtual host. This is a known defect, identified in Defect Work Item #166661 on jazz.net, which will be resolved in the 18.104.22.168 release of Rational Team Concert.
