Error description

• The data warehouse is missing a trigger on
  TESTPLAN_TESTPLAN_HIERARCHY to monitor for changes to the test
  plan hierarchy.  If one child test plan is removed from the
  master test plan, the linking between them can't be removed from
  the TESTPLAN_TESTPLAN_HIERARCHY table in ETL because of the
  missing trigger.
