PM45795: change managment integration documentation is not clear on configuration

Error description

• The customer was trying to setup the integration, but their
  process did not match any of the examples given in the docs.
  Specifically they were concerned about the process names to use
  in the second step of the configuration.
  
  They specifically requested that a statement such as the
  following be added to the manual, but they would also like some
  more information about the general setup i.e. what processes to
  use, and how to find the relevant information on them from
  inside RTC.
  
  'Whether you use the workflow ID or the state ID depends on what
  type of process you are using. A formal project management
  process will require the workflow ID, but a scrum process you
  enter the state itself. '

  better instructions for supported CM systems in the
