Error description

• Under certain scenarios the serialization of a document that
  contained overridden namespace declarations was resulting in a
  not well-formed XML output that had multiple conflicting
  namespace declarations on the same element.

  When requesting the force-ns-declaration on an element that
  redeclares a namespace from its parent's in-scope namespace
  the output will contain both declarations.  This causes the
  output to be not well-formed.
  

Problem conclusion

• The serializer now properly processes the force-ns-declaration
  option.
