Error description
This problem is caused by APAR PK45231 / PTF UK25501. The problem happens on every remote start channel where the transaction definition in the target region specifies ROUTABLE(YES). . The problem is that the call destroy_channel_copy added by PK45231 to DFHEIIC following CALL SHIP_ROUTABLE_START is not always appropriate. In the customer's case and in our recreate, SHIP_ROUTABLE_START does not end up shipping the START anywhere.

Problem conclusion
UK25501 DFHEIIC has been amended such that the channel copy delete, which is done upon return from routine SHIP_ROUTABLE_START, is only performed if the resultant START is to be remote.
