Error description

• The purpose of this APAR is to ship the SMF
  assembler macros for CSQDQSPH (subtype 5),
  CSQDQSGM (subtype 6), and CSQDQSRS (subtype 7)
  that are missing in ACSQMACS and/or SCSQMACS
  libraries.

  libraries.
  
  The C language header CSQDSMFC in libraries SCSQC370 and
  ACSQC370 does include the new SMF 115 subtype definitions.
  

Problem conclusion

• This APAR adds the missing SMF assembler macros to the SCSQMACS
  and ACSQMACS libraries.
