Error description

• Operation name refactoring does not occur under the body level
  in the XML map if the map meets the following condtions:
  1. The XML map which uses "/" as the message root
  2. The above XML map contains local map on the context, headers,
  or body level.
  

Local fix

• There are 3 possible work around to avoid the issue.
  1. Replace the local map with direct mapping.
  2. Replace the message root "/" with body, context, or headers.
  3. If message root "/" and local map are needed, manually update
  the map files (with error marker in IID) after refactoring.
  The operation name (being refactored) under the body tag needs
  to be updated manually.
