Error description

• There is a strange Add action appearing on the Pop-up for a
  Locator Query.  On checking the underlying form and query
  neither contain any Add action, nor is the Show Add checkbox
  ticked, so this Add action has no functionality or can be
  hidden from the end user.
  

Local fix

• No workaround provided.
  

Problem summary

• Need additional checks before displaying an Add action whenever
  a report renders a form.
  

Problem conclusion

• In the Tririga Application, some Add action buttons were being
  displayed to the user even though no Add action was specified
  for that report. This issue has been fixed. Now, an Add action
  would be displayed only if it was defined for this report in
  the Report Manager. This is targeted to the 3.5.1.1, 3.5.0.3,
  and 2h2016 releases.
