  Unique Constraint error when trying to save a workflow revision
  
  Performance Issue: No

  Steps to replicate:
  -------------------
  Go to System Configuration -> Platform Configuration ->
  Workflow Designer
  Create a new WF process, TESTEWF, on object SR (could be any
  but we used the same object as the customer)
  
  Add a positive line between the start and stop nodes
  
  Start ------------------> Stop
  
  On the connector line between Start and Stop open up the
  properties and add an action like Change status (e.g. SR
  INPROG).
  
  In Notifications section of the Action Properties dialog add a
  new row
  In communication template field choose to go to Commnication
  Templates
  Create a new template: 1018
  Applies to: SR
  Sent From: maxadmin
  Subject: test
  On Recipients tab, persons for communication template section
  add a new row
  Person: wilson
  Check the To? checkbox
  Save and activate the template
  Return with value
  
  Click ok to close the Action Properties dialog
  Save, enable and activate the workflow
  
  From select Action menu choose Create Process Revision
  
  Click Save and you will get the unique constraint exception as:
  
  BMXAA4211E - Database error number 2601 has occurred when
  operating on WFNOTIFICATION : Process=TESTEWF Process
  Revision=2 Unique ID=1. Report the error to the owner of the
  deployment.
  
  Expected Result: The revision should be saved
