Abstract
IBM has seen issues with SVC, Storwize® V7000, and Storwize® V7000 Unified during activities that result in path failover. Paths are slandered by the Linux® operating system when running with Qlogic® 8.01.07 or higher and Emulex® 8.16.34 or higher.

If this situation occurs with SVC, Storwize® V7000, and Storwize® V7000 Unified then manually resetting the paths to an online state resolves the issue.
To bring the paths back online manually, check the file named 'model' in all /sys/class/scsi_device/<device dir>/device directories. If 'model' contains 2145, then:
For SLES
Check the file named 'online' in the same directory:
(/sys/class/scsi_device/<device dir>/device/online)
If it contains "0" then the relative path has been slandered so change to "1" by running:
Echo 1 > /sys/class/scsi_device/<device dir>/device/online
For Red Hat
Check the file named 'state' in the same directory:
(/sys/class/scsi_device/<device dir>/device/state)
If it contains "offline" then change to "running" by
Echo "running" > /sys/class/scsi_device/<device dir>/device/state
These issues have been resolved in later levels of Linux, SLES11SP1, and RHEL6.1, when running in combination with the latest drivers.
