Problem Conclusion

This is only a problem on certain Power7 servers, most likely
to occur with R720 TR8 or R730 TR4 and during I/O error
recovery. The partition obtains incorrect DMA address space
information during device recovery. The information has been
corrected. The problem is only known to affect devices on
buses 10, 11, 12, and 13 on these servers. In particular, note
that buses 10 and 12 are only supported in a dual storage
controller configuration, i.e. buses 10 and 12 should be
assigned to a partition together. If only bus 10 or 12, but
not the other, is assigned to a partition, it can cause
internal I/O errors. Without this PTF, that can lead to
partition termination. The likelihood of encountering this
problem is very unpredictable. Customers may wish to avoid
interrupting normal business operations to apply this fix if
the Product Activity Log indicates no recent hardware issues
for I/O devices, on buses 10-13 in particular. Customers on
Power7 servers should not attempt any adapter concurrent
replacement or Dynamic
Logical Partitioning changes until this PTF is applied.
