Error description
DS5k Controller reset (or concurrent FW upgrade) causes expected SC_DISK_PCM_ERR4 errors with ASCQ 9401 (means LUN not owned). However for all new IOs we still use the wrong path and log tons of 9401 errors even for more than 30 min. or until reboot. We simply don't failover to alternate path.
Local fix
Problem summary
When running CCDL htx will return with ENODEV when the FO is attempted.
Problem conclusion
If a disk is partially opened and it attempts to do a FO, the FO will fail. It then marks the hdisk as failing the FO and it sets the FO flags so that some other hdisk can do the FO. The hdisk that is partially opened should not attempt a FO. That hdisk does not know what state the controllers are in. In the FO code, check to see if the disk is partially opened. If it then just stop the FO.
