when an MPIO device has 2 paths and each is using a different adapter and the disk is currently part of A VG that is varied on, if one adapter is placed in a defined state and the max_xfer_sized increased, and cfgmgr run to bring the adapter and path back online, cfgmgr may failed to add the path back into the MPIO device. The error may look similar to this one: [root@lpa-testdhts05:/] # cfgmgr Method error (/etc/methods/cfgscsidisk -l hdisk4 ): 0514-040 Error initializing a device into the kernel.
Local fix
change the max_xfer using the -P flag and reboot

The disk max_tranfer size is increased above the size mentioned in the ODM, when the disk is open. Adding path would fail.
