Error description
In a concurrent Volume Group, if the hdisk's reserve_policy is set to single_path... If the VG is extended to more than one disk with a single extendvg command... Then immediately after executing extendvg on the local node, the VG on the remote node(s) will be immediately forced offline after logging disk access errors. The local node will eventually complete successfully after waiting for ~5 minutes.
