Error description

    * USERS AFFECTED:
    * Systems running the 6100-09 Technology Level with
    * devices.pciex.df1060e214103404.com below the
    * 6.1.9.30 level.
    ***************************************************************
    * PROBLEM DESCRIPTION:
    * In a VIOS environment where you have client LPARs using
    NPIV,
    * Emulex FC/FCoe HBA/CNA 10Gb or higher Fibre Channel adapter,
    * and there are SAN/fabric issues for the NPIV-enabled paths,
    * the VIOS may crash if commands needed for error recovery or
    * the fabric login sequence times out.
    *
    * This has typically been seen when an NPIV client is doing
    * fabric operations such as fabric discovery, relogin after a
    * path failure, or dynamic tracking operations.
    *
    * During these operations some fields in the fscsi driver
    * structures may be erroneously cleared.  Later, when the
    * driver attempts to access these fields, it can result in a
    * system crash, hang, or other unexpected failure.

    * Install APAR IV61759.
