LVM_IO_FAIL entry is reported in errpt when failed I/Os are returned to LVM layer. This entry has errno value indicating the reason for the failure. The reported errno value can be wrong in certain cases.
Local fix
Problem summary
LVM_IO_FAIL errpt entry incorrectly pulls the errno out of the extended buf error field if it's used.
Problem conclusion
Correctly pull the errno to report in LVM_IO_FAIL error.
