Error description
AIX lpar may end up with a crash with stack showing vfc_process_mad. Particularly observed on P9 and above. The AIX vfc client driver did not synchronize the command buffer before sending the cmd address to VIOS vfc-host. Consequently VIOS vfc-host would process a stale cmd and when the response for stale cmd is processed on AIX lpar, it might crash the operating system.
