Business need:
In order to drive the uptake and ongoing use of patient care plans for the management of chronic disease, Precedence Health Care saw an opportunity to develop an automated, collaborative management system, which would deliver better health outcomes for patients.

Solution:
Precedence Health Care worked with IBM to develop the Chronic Disease Management Network (CDM-Net), which allows all caregivers to track and share information, helping ensure the effective delivery of a collaborative treatment plan and, more importantly, a better quality of life for patients.

The Need
In order to drive the uptake and ongoing use of patient care plans for the management of chronic disease, Precedence Health Care saw an
opportunity to develop an automated, collaborative management system, which would deliver better health outcomes for patients.

The Solution
Precedence Health Care worked with IBM to develop the Chronic Disease Management Network (CDM-Net), which allows all caregivers to track and share information, helping ensure the effective delivery of a collaborative treatment plan and, more importantly, a better quality of life for patients. The joint team was able to create an eHealth Bus to underpin CDM-Net, leveraging IBM’s middleware solution for the healthcare industry. IBM’s eHealth Bus concept enables the effective sharing of information from CDM-Net care plans among healthcare participants involved in patients care.
