IBM Dental Basic
IBM Dental Basic is available to all full-time employees at no cost for 2009 (there is a monthly contribution for dependent coverage). The plan covers preventive, diagnostic and basic restorative dental care, such as fillings. There is no coverage for major restorative care such as crowns, root canals and periodontal services, or for orthodontia. Participants have the flexibility to use any eligible dentist but will receive a higher level of benefits if they use a network provider.

For participants who use an in-network dentist, Dental Basic covers preventive care (including up to two routine exams per year) at 100%. For minor restorative care, participants pay 20% of a specially negotiated fee, with no deductible. Those who use an out-of-network dentist or live in an area where a network dentist is not available pay 20% of the usual and prevailing (U&P) rate plus any amount above it for covered preventive and minor restorative care expenses. There is no annual deductible. IBM Dental Basic pays benefits up to an annual maximum of $500 per covered individual, with no lifetime benefit maximum.

IBM Dental Plus
IBM Dental Plus covers a broad range of dental services, including preventive, diagnostic, basic and major restorative care and orthodontia. Participants have the flexibility to use any eligible dentist but will receive a higher level of benefits if they use a network provider.

For participants who use an in-network dentist, Dental Plus covers preventive care (including up to two routine exams per year) at 100%. For all other eligible expenses, participants pay a percentage of specially negotiated fees, with no deductible, as follows:

Those who use an out-of-network dentist or live in an area where a network dentist is not available pay a percentage of the usual and prevailing (U&P) rate plus any amount above it after meeting a $50-per-person annual deductible as follows:

IBM Dental Plus pays benefits up to an annual maximum of $2,000 per covered individual. There is no lifetime maximum benefit except for orthodontic treatment benefits — up to $2,500 per covered individual, which is not counted toward the annual maximum.

CIGNA DMA
With a Dental Maintenance Alternative (DMA), participants receive dental care from dentists and other providers who are part of an organized network. A DMA is similar to an HMO in its managed care approach. It offers lower out-of-pocket costs for most services, with no deductible to meet and no claim forms to file. Participants must select a primary care dentist to coordinate care. Generally, participants are not covered for care received outside the network.

No coverage
IBM employees who decline dental coverage will receive $5 per month in cash. This amount, which is treated as taxable income, will be paid in your semi-monthly paycheck. If you have dental coverage available through a spouse's/domestic partner's employer or another source, you may want to consider this option.

Remember, if you decide not to enroll in dental coverage when you are first eligible, you won't be able to do so during the year unless you have a qualified status change that allows you to enroll.
