The healthcare industry worldwide is undergoing significant change. As healthcare providers look to find efficiencies and improve care, leaders focus on ever more innovative ways of handling patient care across the spectrum that include inpatient and ambulatory scenarios. Driven by government mandates, such as Meaningful Use in the US, and a fundamental shift in payment models from fee-for-service models, to more accountable care organizations, and hospital-physician bundling, the need for integration is never greater. For many, this means unlocking the value of existing clinical applications and data held in disparate Electronic Health Record (EHR) systems, while overcoming IT resource constraints. With existing resources often tied up in lengthy and complex EMR or clinical application implementations, an imperative across all new IT projects is the provision of easy-to-deploy systems that deliver immediate value, often through a staged approach. The rewards for success, however, can be significant in improving the quality of patient care and in delivering services which differentiate the service provided to patients and referring physicians. A key first step, which many organizations are undertaking is the integration of their EMR system with existing clinical applications to ensure that data is captured and stored within the patient records both for immediate observation and later review and analysis. Combining this with data captured at source from bedside medical devices ensures the EMR is accurate and treatment is based upon the most current observations and test results. Directly integrating such applications and devices removes a significant burden on clinicians of re-keying data between systems, reduces the opportunity for errors, ensures data integrity, and removes duplication of pharmacy orders and lab tests. Looking more broadly, the range of new services which can be realized and supported by a robust and scalable integration backbone is significant. For inhouse clinicians, this may include clinical portals for a single view of patient data, whilst for community physicians it may be the ability to order and view laboratory tests or medical imaging. For the patient themselves, this presents a new ability to view and take an active role in their care. It can range from patient portals for streamlined hospital admissions and access to medical records through to in-home tertiary care through the provision of mobile telehealth services. WebSphere Message Broker Connectivity Pack for Healthcare provides ready-to-use support to simplify integration across a range of clinical applications, medical devices, and clinical imaging systems. With a pattern-based development approach, the pack ensures a consistent and repeatable development experience and significantly reduces the time and effort required in creating integrations between such systems. For HL7-based applications, the pack provides a reusable pattern which handles the integration of HL7 v2.x applications, the transfer of data such as patient admissions, discharges, and transfers (ADT), taking and recording observations (ORU), and making orders for prescriptions (ORM). The pattern creates the integration which by designing and handling these flows between applications in a consistent way, can reduce errors due to re-keying and greatly reduce delays in information flow around the hospital. Medical device connectivity is enabled through a series of additional connectors provided within the pack. These provide direct connectivity to a range of medical devices with simple configuration tooling to simplify deployment of many device connections within the hospital environment. The connectors are supported by development patterns which provide rapid integration of the device data with the EMR. WebSphere Message Broker Connectivity Pack for Healthcare V8 adds direct support for the DICOM messaging standard. This provides connectors and supporting patterns which facilitate the creation of integrations between medical imaging systems and image archives. The healthcare sector is one of the biggest generators of Big Data across both clinical and payer scenarios. Historically, the challenge of analyzing this data was identifying the data of interest in vast data warehouses. Integration of the systems using HL7 CDA and related standards for handling patient data provides a new opportunity to gain insight, not retrospectively as before, but based upon the in-flight data as it flows between health systems. WebSphere Message Broker Connectivity Pack for Healthcare V8 provides new tooling to quickly and easily extract a subset of information from CDA clinical documents. With support for a variety of templates which include CDA, C-CDA, CCD, and HITSP, users are able to easily define data elements of interest within a clinical document, in meaningful terms based upon LOINC codes. This non disruptive intelligent filtering supports a wide range of scenarios. WebSphere Message Broker can apply this filter to all messages flowing across the integration backbone, selecting and routing a copy of any matches to the destination application of choice.
